United Airlines Flight 2860 is a scheduled cargo flight from San Francisco, California to Chicago, Illinois, with a mid-sized stop in Salt Lake City, Utah. On December 18, 1977, operated by one Jet Jet flight McDonnell Douglas DC-8-54AF, registration N8047U, the flight crashed into a mountain in the Wasatch Range near Kaysville, Utah. The three crew members, the only passenger of the plane, were killed in the crash.

On December 18, 1977, United Airlines Flight 2860 flew from San Francisco, California to Salt Lake City, Utah. Flights depart from San Francisco at 0017H. When the flight arrived in Salt Lake City at 0111H, the crew radioed the airport that they were having electrical problems, and asked permission to give them time to communicate with the maintenance company. Permission is approved, and the flight enters the holding pattern.

Over the next seven and a half minutes, while in a holding pattern, the flight was absent from the control frequency approach, and entered a dangerous field area. Aviation contact maintenance, and inform them that they are experiencing electrical problems, and that some landing gear lights are not working. After discussing issues with maintenance, and deciding to contact the tower to prepare emergency equipment, they rebuild contacts with towers in Salt Lake City. The controller in charge of Flight 2860, but was unable to contact the flight until re-entering the frequency of the approach. The controller immediately notified Flight 2860 that it was close to the terrain on his right, and instituted the turn left immediately. Did not receive a response, the controller repeated the instruction, which Flight 2860 responded. Fifteen seconds later, the same controller told Flight 2860 to climb up to 8000 feet. Flight reported it rose to 8,000 from 6,000 feet. Eleven seconds later, the flight hit a 7665 foot mountain at 7,200 feet.

The Sheriff's office in Farmington, Utah reported the explosion and subsequent roaring sounds on the ground. The dispatcher called the airport to inquire if any aircraft were missing. The first answer is no. More questions revealed that it was a cargo plane. The Sheriff's Office organizes rescue teams that find corpses and debris. Rescue teams reported that no larger aircraft parts of the suitcase survived the accident. The echoes of the accident can be seen on the slopes for several years afterwards. Witnesses in Kaysville and Fruit Heights saw planes flying low overhead. Soon, all saw an orange light in the east, which continued for three to four seconds. All witnesses reported rain in the area, and some reported it as heavy. The three flight occupants were killed, and the plane was destroyed.

Cause

NTSB concluded that the cause of the accident was "the issuance of controllers and subsequent receipts of the aircraft crew from the holding permit was incomplete and ambiguous". Aircraft crews were cited for their failure to comply with lack of communication guidelines, and lack of compliance with established storage procedures. The electrical problems of the aircraft are referred to as contributing factors.

In addition, the flight cockpit voice recorder is found to be malfunctioning, preventing accident investigation from identifying contributing factors in the cockpit.
